A normal tear film consist of three important compositions:
1. An oily (lipid) composition
2. A watery aqueous composition
3. A mucous-like (mucin) composition
Each composition of the tear films serves an essential purpose. For example, tear lips help keep the tear film from evaporating too quickly and increase lubrication, although mucin can help anchor and spread the tears across the surface of the eye.
Each tear composition is produced by different glands on or near the eye.
The oily composition is created by meibomian glands in the eyelids.
The water composition is produced by lacrimal glands located behind the outer aspect of the upper eyelids.
The mucin composition is produced by goblet cells in the conjunctiva that covers the white of the eye (sclera)
An issue with any of the sources of tear film composition can affect tear stability and causes dry eyes, and there are different types of dry eye deepening on with composition is affected.
An example is if the meibomian gland do not create or secret enough meibum oil, the tear film may evaporate too fast. This condition is called” evaporative dry eye” The primary condition is called meibomian gland dysfunction – today this condition is a significant factor in many dry eye cases.
In other cases, the primary cause of dry eye is that the lacrimal glands were not producing enough water fluid (aqueous ) to keep adequate moisture in the eyes. This condition is known as “aqueous deficiency dry eye.

Health Conditions – Certain systemic disease – such as diabetes, thyroid- associated disease, lupus rheumatoid arthritis and Sjogrens’ syndrome.
Medications – such as prescription and nonprescription medicines, including antihistamines antidepressants, certain blood pressure medications, and birth control pills.
Eyelid problems – incomplete closure of the eyelids when blinking or sleeping – a condition attributed to as lagophthalmos, which can be caused by aging or occur after cosmetic blepharoplasty or other reasons – can cause severe dry that can lead to a corneal ulcer if not treated.
