The tick paralysis is an ascending flaccid paralysis, which occurs when trapped toxic Ixodidae (hard ticks) for several days.

In North America, some Dermacentor can – and Amblyomma species cause tick paralysis by donating via saliva a neurotoxin. The toxin is in the first phase after the bite in which the blood is sucked, not present in the saliva of the tick. This means that the paralysis only occurs when the tick has been stuck for several days and has become saturated. A single bite can cause paralysis then, especially if the tick is stuck on the back at the top or near the spine.

Symptoms include anorexia, lethargy, muscle weakness, incoordination, nystagmus and an ascending flaccid paralysis. It can lead to bulbar paralysis and respiratory paralysis. Diagnosis is based on clinical findings. A tick paralysis should be considered in North American patients with acute flaccid paralysis or increasing bulbar paralysis; it should be sought on the entire body for ticks and removing them. The differential diagnosis one must think of Guillain-Barre syndrome, botulism, myasthenia gravis, hypokalemia and spinal cord tumors. The tick paralysis can be fatal, but goes after the removal of the tick (s) back quickly. The paralysis usually begins a few hours after tick removal, but they may continue for 24 to 48 hours after removing ticks. If breathing is restricted, O2 administration and artificial respiration may be necessary.
