A nutritional niacin deficiency, pellagra caused hardly occurs in industrialized countries. Clinically it is manifested in the “three Ds”: localized, pigmented rash (dermatitis), gastroenteritis (diarrhea) and wide-ranging neurological deficits, including the decline of cognitive abilities (dementia). The diagnosis is usually made clinically, a dietary supplement of the nutrient (orally or if necessary i.m.) usually eliminated the defect.

To the niacin (nicotinic acid) include derivatives of nicotinamide adenine dinucleotide (NAD) and nicotinamide adenine dinucleotide phosphate (NADP), which act as coenzymes in redox reactions. They are essential necessary for cell metabolism.

Etiology A primary niacin deficiency caused by an extreme undersupply of both niacin and tryptophan, which is typical of areas where the population mainly feeds on corn. Bound niacin contained in corn is only assimilated in the gastrointestinal tract when it was previously treated with alkali, as is done in the preparation of tortillas. Corn protein does not contain enough tryptophan. The frequent occurrence of pellagra among people in India who eat millet high leucine content has led to the assumption that an unbalanced budget amino acids contribute to niacin deficiency. A protein deficiency and an insufficient supply of many B vitamins often accompany a primary niacin deficiency. A secondary niacin deficiency may be caused by diarrhea, cirrhosis or alcoholism. Pellagra can (is redirected to form 5-hydroxytryptophan and serotonin tryptophan) and occur in the Hartnup Disease (absorption of tryptophan by the intestine and the kidney is impaired) .Symptome and discomfort pellagra manifests itself in symptoms even when carcinoid syndrome the skin, the mucous membranes, of the CNS and of the gastrointestinal tract. Advanced pellagra caused a symmetrical light-rash, stomatitis, glossitis, diarrhea and mental confusion. The symptoms can occur individually or together. Skin symptoms consist of various types of lesions that occur symmetrically on both sides. The distribution of the lesions – exposed to bruising or sun-damaged skin areas – is more characteristic of the disease than their form. Lesions may develop (pellagra glove) or boat-shaped distribution on the feet and legs (pellagra boot) in glove-like distribution on the hands.
