Gastric Cancer

The etiology of gastric cancer is multifactorial, but Helicobacter pylori plays a significant role. The symptoms are earlier saturation, obstruction and bleeding, but they tend to occur late in the disease process. The diagnosis is made by endoscopy, followed by CT and endoscopic ultrasound for staging. Treatment is primarily surgical; chemotherapy can provide a temporary response. The long-term survival is poor, except for patients with only local disease.

Gastric cancer in the US annually responsible for approximately 22,200 cases and 11,000 cancer deaths. The gastric adenocarcinoma is responsible for 95% of malignant tumors of the stomach; rare are localized gastric lymphomas (Lymphomas Overview) and leiomyosarcoma. Gastric cancer is the second most common cancer, but the incidence varies greatly; it is extremely high in Japan, China, Chile and Iceland. In the US, the incidence has fallen in recent decades, it is now the seventh leading cause of cancer death. In the US, it is most common among blacks, Latinos and Indians. Its incidence is increasing with age,> 75% of patients are> 50 years.

Etiology The cause of most gastric cancers is a Helicobacter pylori infection Autoimmune atrophic gastritis (autoimmune metaplastic atrophic gastritis) and various genetic factors (Gastrointestinal stromal) are also risk factors. Diet-related causes, however, are not detected. Gastric polyps can be precancerous. In patients taking NSAIDs, is inflammatory polyps may form foveoläre fundus polyps are often taking proton pump inhibitors in patients. Adenomatous polyps, v. a. when they occur multiple, have the greatest chance to develop into a carcinoma, even though they are rare. A carcinoma is v. a. probably when an adenomatous polyp has a diameter> 2 cm or histologically villous structure. Because malignant transformation can not be detected by a macroscopic inspection, all should be removed discovered during endoscopy polyps. In patients with duodenal ulcers, the incidence of stomach cancer has decreased in general. Pathophysiology adenocarcinomas of the stomach can be classified macroscopically as follows: projecting: The tumor is polypoid or mushroom-populated. Penetrating: The tumor is ulcerated. Superficially spread: The tumor spreads along the mucosa or infiltrate superficial gastric wall. Linitis plastica: The tumor infiltrated the stomach wall with an accompanying fibroid reaction which leads to a rigid “leather bag” -Stomach. Mixed: The tumor exhibits characteristics of at least two of the other types; this is the largest group. Projecting tumors have a better prognosis than flatly spreading, as they used to cause discomfort. Gastric cancer imaging provided by David M.
