Fingerbeugertendinitis And Tenosynovitis

(Trigger finger)

This often idiopathic inflammation affects the tendons and tendon sheaths of the finger flexors, often resulting in fibrosis.

In RA and diabetes mellitus this inflammation occurs very frequently. Repetitive use of the hands (as with the use of a heavy secateurs) may contribute to the disease. In diabetic patients coexistence may be with carpal tunnel syndrome and occasionally fibrosis of the palmar fascia. The pathological changes begin with a thickening or knot formation in the tendon, the occurrence in the field of natural annular constrictions of the tendon sheaths, the stretching or flexing of the finger will be blocked. It is the phenomenon of “trigger finger” with blockade at diffraction and sudden release (trigger) of the extension with a snapping noise.

Therapy Conservative measures Occasionally Kortikosteroidinjektion Occasionally surgery Treatment of acute inflammation and pain includes splinting, moist heat and anti-inflammatory doses of NSAIDs (Rheumatoid arthritis (RA): NSAIDs). this is not sufficient, the injection of a Kortikosteroidsuspension into the tendon sheath along the splinting for secure rapid improvement of pain and blocking can provide. Surgical treatment may be attempted if the corticosteroid therapy failed.
