Boutonniere Deformity

(Boutonnière deformity)

It is a deformity of the proximal interphalangeal with diffraction (PIP) joint and overextension in the distal interphalangeal (DIP) joint (s. Buttonhole and Schwanenhalsdeformitäten.).

The boutonniere deformity caused by tendon rupture, dislocation, fracture, arthritis or RA. In the classical form the deformity caused by an interruption of the central portion of the approach of the extensor tendon to the base of the middle phalanx, whereby the proximal end of the proximal phalanx between the lateral band portions passes as through a button hole. Treatment initially consists of splinting, but it must begin before they develop scarring and fixation of the deformity. A surgical reconstruction can often no longer produce normal motility, or at least reduce the deformity and improve the function of the hand.
