Blastomycosis

(Gilchrist’s disease; North American blastomycosis)

Blastomycosis is a lung disease caused by the inhalation of spores of the dimorphic fungus B. dermatitidis. Occasionally, the fungus spread hematogenous what causes extrapulmonary disease. The symptoms are caused by pneumonia or dissemination of multiple organs, most commonly the skin. The diagnosis is clinically detected by chest X-ray, or both and confirmed by laboratory evidence of the fungus. The treatment is carried out with itraconazole, fluconazole or amphotericin B.

In North America, the endemic area for blastomycosis includes the Ohio-Mississippi -Flusstäler (which has become the central Atlantic and southeastern states spread) the Northern Midwest the New York State southern Canada rarely occurs infection also in the middle East and Africa. Immunocompetent people may become infected. Although blastomycosis in immunocompromised patients is more common and severe, it is a less common opportunistic infection than histoplasmosis or coccidioidomycosis. B. dermatitidis grows as a mold at room temperature in exkrethaltigem soil and in wet, rotting, acidic organic material, often near Rivers. In the lungs inhaled spores invasive yeasts are transformed into large (15-20 microns), causing the characteristic broad-based buds. Once in the lungs, an infection can be confined to the lungs Hematogenous disseminiern Hematogenous dissemination can be a focal infection in numerous organs, incl. The skin, prostate, epididymis, testes, kidneys, vertebral bodies, terminations of long bones, subcutaneous tissue, the brain, oral or nasal mucosa, thyroid, lymph nodes, and bone marrow and other tissues. Symptoms and complaints lung Pulmonary blastomycosis may be asymptomatic or cause an acute, self-limiting disease that is often not noticed. But it can also begin insidiously and progress to a chronic, progressive infection. The symptoms consist of a productive or dry, staccato cough, chest pain, dyspnea, fever, chills and severe sweating. Occasionally there may be a pleural effusion. Some patients have rapidly progressing infection, and it may be a respiratory distress syndrome (ARDS) entwickeln.Extrapulmonal When extrapulmonary disseminated blastomycosis the symptoms of involved organs depend. Skin lesions occur far the most common, may occur singly or multiply and run with or without a clinically apparent pulmonary involvement. On exposed surfaces there is usually papules or papulopustules that spread slowly. At the spreading boundaries large painless, miliary abscesses that spread slowly develop pinhead to 1 mm. On irregular surfaces, wart-like papillae can form. Sometimes blisters develop. The lesions enlarge, while the center heals atrophic scars from forming. When fully developed, an individual lesion appears as a raised verrucous patch, usually ? 2 cm in diameter with a purple, abszessbesetzten border. For an existing superinfection may cause ulceration.
